Rosie O'Donnell shared new insight on what she was told before deciding to undergo a lower facelift
The comedian revealed on Watch What Happens Live with Andy Cohen that she was advised to get a surprising procedure done
O'Donnell announced her secret facelift in May, four months after she got it done in January
Rosie O'Donnell wouldn't want her transformation to look any other way.
The comedian, 64, has been open about the details of her recent plastic surgery — and now the enhancements she wanted to avoid.
During her appearance on Watch What Happens Live with Andy Cohen, the former talk show host — who announced in May that she underwent a secret lower facelift, done from the lips down, in January — revealed what sort of recommendations she received while shopping for procedures.
Host Andy Cohen praised O'Donnell for not wanting to overdo the tweaks, saying, "I also like that you didn't say, 'You know what do my lips.'"
She responded, "Some of the ones I went to said —I go, 'What do you think I should do? I just want to look not so sad.' One guy had like 26 things. He wanted to put wire in my head then pull up my cheeks. I said no, no, no, no."
O'Donnell joked that she wouldn't have agreed to a full makeover. However, she's more than happy detailing her recent procedure, which only altered a portion of her face.

Details on O'Donnell's facelift were first shared in a Substack essay, in which she wrote that the surgery "cost more money than I have ever paid for a car." And while she took accountability for previously touting herself as anti-facelift — she wrote, "I thought it was a betrayal. Of feminism. Of aging. Of our team of women worldwide…" — it was her weight loss that inspired her to go for it (the former View host has lost over 60 lbs. through surgery, lifestyle changes and weight loss drugs over the years).

By accepting her desire to undergo a cosmetic transformation, she also hoped to send a powerful message to her five children. "I want them to grow up in a world where they don't feel like they have to change but also knows they can, if they want to, without losing moral standing in their own life."
When making her first public appearance since sharing intimate details of her transformation at the 2026 Tonys, O'Donnell spoke with E! News about why she wanted to be so forthcoming about her refreshed look.
"All that matters is truth and love," she shared. "And so I wanted to be truthful and say all the complicated emotions I had about it. I just felt it was better to be truthful than not and I didn't want some tabloid to go, 'Gotcha!'"
